Annapurna Scheme’s 11-Page Form Released, Applicants Must Submit Details from Land Records to Bank Accounts
India News : The West Bengal government has released an extensive 11-page application form for the Annapurna Scheme, under which eligible women will receive ₹3,000 per month as financial assistance.

Chief Minister Suvendu Adhikari announced the rollout of the form during a press conference at Nabanna, stating that the application process will begin on June 1 and continue for the next three months.

Extensive personal and family details required

The application form seeks detailed information about the applicant and every member of the family. Applicants must provide:

  • Name of the head of the family
  • Date of birth
  • Address
  • Aadhaar number
  • Aadhaar-linked mobile number

The form also requires details of all family members, including their relationship with the applicant, Aadhaar details, and other personal information.

Bank account and voter details mandatory

Applicants must submit the bank account details of every adult family member. The government has made it compulsory for bank accounts to be linked with Aadhaar.

In addition, the form asks for:

  • Voter ID card numbers
  • Assembly constituency details
  • Part numbers of voter lists
  • Information related to digital ration cards, if available

Property and land information required

The scheme application also demands detailed property-related information. Families must disclose:

  • Whether they own a permanent house
  • Details of land ownership
  • Mutation and registration documents of land
  • Ownership of four-wheel vehicles by any family member

Income, education and welfare scheme details

The form further asks whether the family is already benefiting from any state government schemes.

Applicants must also provide:

  • Health insurance details
  • PAN card information
  • Occupation and income-related documents
  • Educational qualifications and certificates of all family members

Citizenship and social data questions included

The form includes several additional questions related to:

  • Annual family income
  • Benefits under schemes like Student Credit Card
  • Citizenship applications under CAA
  • Names removed under SIR
  • Applications before tribunals

Information about children’s schools and vaccination status has also been sought.

Online and offline application process

According to the Chief Minister, applications can be submitted both online and offline. The government has also said that officials will visit homes to assist people who are unable to fill out the forms themselves. Legislators have also been directed to help with the process.

Government says data will help future schemes

The state government stated that the purpose of the detailed form is to create a comprehensive social and economic database of families, which could also be used for future welfare schemes.

Opposition raises concerns

Opposition parties and many citizens have criticized the lengthy documentation process, saying that demanding such a large amount of information and paperwork may make it difficult for ordinary people to apply for the scheme.
